We are recruiting an administrator to join our team. The main responsibility of the scheduled care co ordinator is to identify and book patients for chronic disease reviews with the most appropriate clinician(s) in order to streamline their annual reviews and reduce the number of presentations to the practice for planned care.
An understanding of the Quality Outcomes Framework (QOF) and SystmOne clinical system would be desirable, however full training will be provided. The role is part of the administration team striving to be a centre of excellence for patient care.
Location: Brighton.
Responsibilities
- Identify patients who have opted in or out of the scheduled care protocol.
- Use Mjog to identify patients who have responded to the recall message sent.
- Book patients with appropriate clinicians and timing, aiming to have their medication review completed by the end of the month of their birth.
- Respond to patient queries regarding long term condition checks, directing to clinicians where appropriate.
- Use AccuRx and SystmOne messaging as per the scheduled care protocol.
- Use ICE to raise blood forms as per the Long Term Condition Ardens template.
- Perform audit work where required.
- Liaise with the Operations Manager, Data Quality Manager and clinical leads where necessary.
