An exciting opportunity has become available in our Diabetes Nursing Team.

As a key member of the Adult Diabetes Service, you will provide technical support and education to people with diabetes (PWD) using all forms of diabetes technology.

The role is key to increasing digital access for all our patients and rolling out hybrid closed loop technology.

This post holder will support the Diabetes Specialist Nursing (DSN) to implement Hybrid Closed Loop insulin pump systems within the adult service with an aim to reducing health inequalities and improving health outcomes.

Experience of Diabetes Technology initiation including HCL systems is desirable for this role.

• Support the DSNs with insulin pump training for PWD starting pumps, upgrading their system and initiating hybrid closed loop systems.
• Demonstrate and support the setup of glucose monitoring systems, associated readers, smartphone apps and online platforms as needed.
• Facilitate or supervise the insertion of sensors / sub cutaneous cannulas via sterile procedure.
• Support the PWD to download and synchronise devices for sharing clinical information.
• Support diabetes HCL clinics to ensure PWD attending have successfully shared clinical data prior to review.
• Provide support with problems with diabetes technology via telephone, email, video call or face to face.
• Within competency, recognise abnormalities in data uploaded from insulin pumps and escalade to registered nurse for management.
• Coordinate the HCL pathway, working closely with the Diabetes Nurse team administrator, including booking of group sessions and ordering of appropriate equipment via SBS system prior to HCL initiation sessions if needed.
• Work closely with external insulin pump company representatives to facilitate education sessions.
• Participate in clinical discussions/meetings regarding patients' care and work collaboratively with a range of professionals in statutory and non statutory organisations and agencies.

• Maintain high standards of care ensuring that all procedures and guidelines are followed.
• Following training and assessment of competence undertake practical clinical tasks as designated by the DSNs.
• Contribute to insulin pump starts by ensuring the PWD is familiar with the technological aspects of their device.
• Working with the diabetes nurse team administrator, contribute to processes prior to pump start to ensure all necessary equipment is available on the day of pump initiation.
• Support downloading of necessary apps/software and provide ongoing support and follow up post pump start to ensure connectivity.
• Understand menu options for each device to support PWD to use the necessary features.
• Support download of devices during HCL clinics if required.
• Support PWD to continue using diabetes tech during hospital admissions in the inpatient setting.
• Perform physical tasks such as blood glucose and ketone monitoring and interpret readings.
• Be competent to take venous blood specimens for annual review blood tests if required.
• Support DSN led education sessions.
• Deal directly with queries from patients and other health care professionals and refer to appropriate diabetes team member where necessary.
• Demonstrate motivational and reassuring communication skills to support patients through significant changes in their diabetes treatment, recognising that such transitions can trigger challenging behaviours, frustration, and heightened anxiety.
• Demonstrate a caring approach to PWD. Have good verbal and non verbal communication skills. Acts as an advocate for PWD at all times.
• Recognise and maintain confidentiality of information in all communication including online media relating to patients/clients and their records.

• Foundation Degree in Health and Social Care (Assistant Practitioners)
• Nursing Associate Foundation Degree (including registered on NMC Register - Nursing Associates)
• Venepuncture and cannulation competency
• Blood glucose and ketone testing competency

• Exposure to a variety of clinical settings
• Experience of working with PWD in clinical settings
• Experience of working within an MDT
• Strong IT skills, including Microsoft office packages, specifically EXCEL
• Comprehensive understanding of app based technology
• Experience of diabetes technology initiation including HCL systems
• Experience in using Oracle SBS IT package
